Patient Travel Letter Request Form

If you are travelling for longer than a month, and require a longer prescription, we may need to request proof of travel from you.

As travel letters as not an NHS service, we must charge a processing fee for them, whether you are a private or NHS client of ours. The charge is £20, and your travel letter will not be sent until we receive this payment.

Kindly pay with the same email address we have on file for you, so we can track your payment smoothly, and avoid any delays.

Please give 7-10 days’ notice for travel letters, where you can. We cannot guarantee you will receive your letter in time, if payment is made after that time frame.
